PAO and surgical dislocation for a “Perthes” deformity in a 16 year old soccer athlete

History and Evaluation: This 16 year old female, soccer athlete was treated for Perthes disease seven years previously with a femoral osteotomy.  She presented to us with lateral hip and deep groin pain that was worse with activities.

X rays revealed a “Perthes-like deformity” with a large aspheric femoral head, high trochanter, short, wide femoral neck and associated acetabular dysplasia (shallow socket).

Treatment: She was treated with a combination of surgical dislocation and periacetabular osteotomy (PAO).  The surgical dislocation and femoral osteochondroplasty treated the impingement and the periacetabular osteotomy corrected the acetabular dysplasia.

Outcomes: One year after surgery the patient had her hardware removed. She has an excellent clinical result and has returned to soccer.
