History and Evaluation: This 29 year old female presented with a six month history of dull, aching and sharp groin pain. The pain was severe and affected her ability to do daily and recreational activities. She had been diagnosed with Perthes disease ten years earlier but did well and underwent no treatment at that time.
X Rays show an aspherical Perthes deformity of the femoral head, a short femoral neck, high trochanter and acetabular dysplasia (shallow socket) on the right side.
Treatment: Surgical treatment included a periacetabular osteotomy (PAO) to treat hip dysplasia (shallow socket), and a surgical dislocation to reshape the Perthes like femoral head and advance the greater trochanter.
Outcomes: Two years after surgery patient has an excellent clinical result, no pain and has returned to recreational running.
